Equitrans Midstream is planning an improvement to the process of customer onboarding. We are automating our customer onboarding processes effective
November 15, 2021. All new user access requests to our Gas Management System (QPTM/TIPS/Measurement Portal) will be initiated and processed online
via our new Quorum Online Onboarding Process. PDF documents will no longer be accepted.

• New Service: The Local Security Administrator (LSA) will answer a series of questions, the answers to which will result in a new legal Interstate, Gathering
or Water Customer Activities Website (CAW) Agreement. Signatures will be captured online replacing the need for pen, paper, and scanning. Upon approval
logon credentials will be generated for individuals specified on the Agreement(s).

• New Individual User: An individual requesting access under an existing CAW Agreement will answer questions which, upon approval, will result in the
generation of logon credentials.
